Minimally invasive treatment of acute and chronic mixed traumatic hematoma -faster and good effect

目的: To investigate the clinical curative effect of acute and chronic trauma mixed hematoma(ACMTH) with minimally invasive treatment.
方法:

78 cases were treated with minimally invasive drainage using YL-1 puncture needles and amended twist drill craniostomy. The operative timing, effect and postoperative complications ,GOS score were evaluated.

结果:

Male35 cases, female43 cases among 78 patients. The average age was 65.1±10.2. GCS score at admission was 9.5±1.5. First CT showed that chronic subdural hematoma was 125.5±30.2ml, and epidural hematoma was 25.2±8.1ml. Preoperative epidural hematoma was 40.3±5.0ml. Operation timing was 11.5±3.5 hs after admission. Minimally invasive lavage and drainage of hematoma with YL-1 needles under CT location. Average operation time was 20.2±5.1mins. After operation, 2 times urokinase were injected to epidural hematoma cavity perday. Chronic subdural hematoma was 60.1±10.3ml (P < 0.01) at 24 hs postoperation. Epidural hematoma was 20.1±5.0ml (P < 0.05) at 24 hs postoperation. Hematoma clearance time was 4.1±1.1 ds. Postoperative 24h GCS was 11.1±1.0, GCS was 12.5±1.1 at 3 days postoperation significantly higher than preoperation (P < 0.05). 3 months GOS score was 5 in 63 cases (81%), 4 in 12 cases (14%), 3 in 2 cases (2%), and 2 in 1 (1%) patient.

结论: Minimally invasive treatment of ACMTH is fast and effective.
