The resarch clinical efficacy and safety of domestic 8Spheres polyvinyl alcohol embolization microspheres combined with 1000 μ gelatin sponge embolization by arterial infusion chemotherapyin the treatment of large hepatocellular carcinoma.

Objective:To observe the short-term clinical efficacy and safety of domestic 8Spheres polyvinyl alcohol embolization microspheres combined with 1000 μ gelatin sponge embolization by arterial infusion chemotherapyin the treatment of large hepatocellular carcinoma.
Method :From May 2016 to November 2017, 58 patients with primary liver cancer who could not be resected or not underwent radical resection in our hospital were retrospectively analyzed. The patients were divided into the observation group (8Spheres microspheres +1000 μ gelatin sponge) in 32 cases, the control group (8Spheres microspheres) in 26 cases, and the two groups were treated with TACE.
The perioperative and postoperative follow-up clinical data, the maximum dilution value of alpha fetoprotein (AFPmax) and liver function indexes of the two groups were compared and analyzed.The local response and postoperative complications of the two groups after operation were compared by CT or MR enhancement.
Result:54 operations were performed in the observation group and 49 operations were performed in the control group. The successful rate of operation was 100% in the two groups. The median follow-up time was 9.5 months(4-15 months).There was no significant difference in baseline characteristics between the two groups (P > 0.05).
According to the standard of mRECIST modified solid tumor evaluation, the effective rate(RR) of the observation group in the 3rd and 6th month was 79.2%.and 68.9%,the disease control rate (DCR) was 89.3% and 71.0%.The RR of the control group was 70.3% and 61.5%.DCR was 76.0% and 65.9%.The median time of disease progression(mTTP) was 8.1 months. The control group was 6.2 months.The observation group was higher than the control group, and the difference was statistically significant (P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups ( P < 0.05 ) .
Conclusion :Compared with the simple 8Spheres microspheres, the domestic 8Spheres microspheres combined with 1000 μ gelatin sponge artery embolization for the treatment of large hepatocellular carcinoma has better clinical efficacy and safety in the near future, which has clinical promotion value.However, the long-term clinical effect and survival benefit still need multi center and large sample follow-up observation.
