LifePort machine perfusion versus cold storage in kidney transplantation: a meta-analysis of randomized controlled trials

目的:
Static cold storage is generally used to preserve kidney allografts from deceased donors. Hypothermic machine perfusion by LifePort may improve outcomes after transplantation, but few sufficiently powered statistic evidence has addressed this possibility. The aim of our study was conducting a Meta-analysis of Randomized controlled trials(RCT)to compare the functional results of renal preservation in hypothermic machine perfusion (HMP) versus cold preservation (CP).
方法: Pubmed, Embase and Web of Science were searched for RCTs comparing HMP with CP in renal transplantation. The quality of included studies was assessed and Meta-analysis was conducted by Review Manager 5.3 software.

结果:  Eight randomized controlled trials including 2313 allografts met the inclusion criteria. The delayed graft function(DGF) of HPM group was found inferior to the CP group (Risk Ratio (RR)=0.77, 95%CI [0.68-0.87], p<0.0001). HPM also improved functional delayed graft function (RR=0.66, 95%CI [0.59-0.79], p<0.00001) and primary non-function (RR=0.40, 95%CI [0.23-0.70],p=0.001). But the acute rejection rates were statistically similar in the two groups (RR=0.93, 95%CI [0.72-1.21],p=0.60). 

结论:
HMP improved the DGF, fDGF and PNF results in grafts from cadaver donors of all types, although more clinical trials are needed to futher prove that.
