The application of LifePort system in preservation of kidney donated after cardiac death

目的:
Hypothermic machine perfusion by LifePort may improve outcomes after DCD renal transplantation, but few sufficiently powered statistic evidence has addressed this possibility. The aim of our study was conducting a Meta-analysis of Randomized controlled trials(RCT)to compare the functional results of renal preservation with hypothermic machine perfusion (HMP) in LifePort system versus traditional cold preservation (TCP).
方法:
Pubmed, Embase and Web of Science were searched for RCTs comparing HMP with TCP in renal transplantation. The quality of included studies was assessed and Meta-analysis was conducted by Review Manager 5.3 software.
结果:  Five randomized controlled trials including 746 allografts met the inclusion criteria. The delayed graft function(DGF) of HPM group was found inferior to the TCP group (Risk Ratio (RR)=0.76, 95%CI [0.65-0.88], p=0.0002). HPM also improved functional delayed graft function (RR=0.51, 95%CI [0.30-0.89], p=0.02) . But the primary non-function rate (p=0.68) and acute rejection rate (p=0.05) were statistically similiar in the two groups.

结论: HMP improved the DGF and fDGF results in grafts from donors after cardiac death, although more clinical trials are needed to futher prove that.
